Patient presentation and recorded decisions

Assessment & Treatment Planning

Patient goal: Reconstruct the frontal hairline and improve visible coverage across the frontal and mid-scalp regions while maintaining a treatment plan consistent with facial proportions and available donor capacity.

Assessment context: Advanced frontal recession, significant bitemporal thinning and reduced visible density across the mid-scalp region.

Planning rationale: A combined Sapphire FUE and DHI approach was selected because the treatment plan involved both frontal hairline refinement and broader frontal and mid-scalp coverage. FUE-based extraction was used for follicular-unit harvesting, while recipient-site and implantation workflows were selected according to the requirements of different treatment zones rather than applying one implantation method uniformly across the entire recipient area.

Design notes: The frontal hairline was planned according to the patient's facial proportions, existing recession pattern and Afro-textured hair characteristics. The anterior boundary was designed to remain proportionate while graft direction and distribution were planned around the patient's natural curl pattern and growth orientation.

Recorded Procedure

Follicular units were harvested from the posterior scalp donor region using FUE-based extraction with planning adapted to Afro-textured follicular curvature. Across the planned recipient areas, Sapphire recipient-site creation and DHI implantation workflows were used according to the treatment design, with graft placement adapted to the intended direction, angle and distribution across the hairline, frontal region and mid-scalp.

Clinical Interpretation

Documented Observation

The pre-treatment scalp record describes advanced frontal recession, significant bitemporal thinning and reduced visible density across the mid-scalp region. The recorded treatment field was the frontal hairline, frontal scalp and mid-scalp. The recorded procedure used 3,600 grafts. At 12 months, the clearest visual change is concentrated at the frontal hairline, with additional photographed change through the frontal scalp and mid-scalp. The treatment did not target the crown, so these photographs should not be interpreted as evidence of crown restoration. Because tight curl can increase the apparent coverage seen in two-dimensional photographs, the images are a visual record of this patient's result rather than a direct measure of density. This is the documented result of one patient and does not predict another patient's outcome.
