Patient presentation and recorded decisions

Assessment & Treatment Planning

Patient goal: Restore a natural, mature frontal hairline and improve visual coverage across the mid-scalp and crown while preserving limited scalp donor reserves.

Assessment context: Advanced male-pattern hair loss consistent with Norwood IV/V, with significant frontal hairline recession, deep bitemporal thinning and severe diffuse density loss extending through the forelock, mid-scalp and crown.

Planning rationale: A body hair transplant strategy using Sapphire FUE was selected because scalp donor availability alone was limited relative to the planned treatment area. Of the 4,600 grafts transplanted, 3,600 were harvested from the scalp donor area and 1,000 from the submandibular beard region. Finer scalp grafts were prioritized for the frontal hairline, while beard grafts were used selectively within interior recipient zones to supplement coverage while conserving scalp donor supply.

Design notes: A mature, anatomically balanced frontal hairline was designed according to the patient's facial proportions and existing hair-loss pattern. Single-hair scalp grafts were prioritized along the leading edge to create a softer transition. Larger scalp follicular units and supplementary beard grafts were distributed behind the frontal zone and through selected mid-scalp and crown areas according to the treatment plan.

Recorded Procedure

A total of 4,600 follicular-unit grafts were transplanted using FUE extraction: 3,600 grafts from the occipital scalp donor area and 1,000 grafts from the submandibular beard donor area. Recipient sites were created using sapphire blades according to the planned direction, angle and zone-specific distribution. Scalp and beard grafts were then allocated according to their characteristics and the requirements of each recipient zone.

Clinical Interpretation

Documented Observation

The pre-treatment scalp record describes advanced male-pattern hair loss consistent with Norwood IV/V, with significant frontal hairline recession, deep bitemporal thinning and severe diffuse density loss extending through the forelock, mid-scalp and crown. The recorded treatment field was the frontal hairline, temporal corners, mid-scalp and crown. The recorded procedure used 4,600 grafts. At 12 months, the clearest visual change is concentrated at the frontal hairline, with additional photographed change through the temporal corners and mid-scalp. The crown was included in the treated field; the photographs record that zone as treated and should be read zone by zone, without assuming the same visual change in every area. In the follow-up views, the transplanted areas appear visually continuous with the patient's existing wavy hair pattern. This is the documented result of one patient and does not predict another patient's outcome.
