Patient presentation and recorded decisions

Assessment & Treatment Planning

Patient goal: Lower and soften the frontal hairline while improving visible coverage across the frontal scalp and temporal regions in a way that remains proportionate to the patient's facial features and available donor capacity.

Assessment context: High frontal hairline recession, temporal thinning and reduced visible density along the anterior margin.

Planning rationale: A combined Sapphire FUE and DHI approach was selected because the treatment plan involved both broader frontal coverage and detailed placement along the hairline and temporal regions. FUE-based extraction was used for follicular-unit harvesting, while Sapphire recipient-site creation and DHI implantation workflows were applied according to the requirements of different recipient zones.

Design notes: The frontal hairline was planned according to the patient's facial proportions and treatment goals, using soft contours and subtle irregularity rather than a rigid linear border. The temporal transitions were designed around the natural curl pattern, direction and exit orientation of the patient's Afro-textured hair.

Recorded Procedure

Follicular units were harvested from the posterior scalp donor area using FUE-based extraction with planning adapted to Afro-textured follicular curvature. Sapphire recipient sites were created across selected frontal recipient zones, while a DHI implantation workflow was used for planned hairline and temporal placement. Graft direction, angle and distribution followed the treatment design and the patient's existing hair pattern.

Clinical Interpretation

Documented Observation

The pre-treatment scalp record describes high frontal hairline recession, temporal thinning and reduced visible density along the anterior margin. The recorded treatment field was the frontal hairline, temporal corners and frontal scalp. The recorded procedure used 2,850 grafts. At 12 months, the clearest visual change is concentrated at the frontal hairline, with additional photographed change through the temporal corners and frontal scalp. The treatment did not target the crown, so these photographs should not be interpreted as evidence of crown restoration. Because tight curl can increase the apparent coverage seen in two-dimensional photographs, the images are a visual record of this patient's result rather than a direct measure of density. This is the documented result of one patient and does not predict another patient's outcome.
