Patient presentation and recorded decisions

Assessment & Treatment Planning

Patient goal: Reconstruct an age-appropriate masculine frontal hairline, improve visible coverage through the bitemporal recession areas and increase coverage across the thinning forelock and anterior mid-scalp.

Assessment context: Marked frontal hairline recession, deep bitemporal recession and diffuse thinning through the central forelock and anterior mid-scalp, documented in a Norwood 3V to 4 pattern.

Planning rationale: Sapphire FUE was selected for a treatment plan requiring FUE-based graft extraction followed by recipient-site creation across the frontal and anterior mid-scalp regions. Sapphire blades were used for planned site creation, with angle, direction, depth and distribution determined according to the patient's existing hair pattern and treatment design.

Design notes: The frontal hairline was planned according to the patient's facial proportions, age and treatment goals, using a mature masculine contour with subtle irregularity rather than a rigid linear border. Single-hair follicular units were prioritized along the leading edge, with multi-hair grafts distributed behind the transition zone and through the forelock according to the treatment plan.

Recorded Procedure

Follicular units were harvested from the occipital donor region using FUE-based extraction. Recipient sites were created across the planned frontal hairline, bitemporal and anterior mid-scalp regions using sapphire blades, followed by graft placement according to the planned angle, direction and distribution.

Clinical Interpretation

Documented Observation

The pre-treatment scalp record describes marked frontal hairline recession, deep bitemporal recession and diffuse thinning through the central forelock and anterior mid-scalp, documented in a Norwood 3V to 4 pattern. The recorded treatment field was the frontal hairline, temporal corners and mid-scalp. The recorded procedure used 3,600 grafts. At 12 months, the clearest visual change is concentrated at the frontal hairline, with additional photographed change through the temporal corners and mid-scalp. The treatment did not target the crown, so these photographs should not be interpreted as evidence of crown restoration. This is the documented result of one patient and does not predict another patient's outcome.
