Patient presentation and recorded decisions

Assessment & Treatment Planning

Patient goal: Reconstruct an age-appropriate masculine frontal hairline and improve visible coverage across the frontal third, mid-scalp and crown while working within the available donor capacity.

Assessment context: Pronounced frontal hairline recession, deep bitemporal recession and diffuse thinning extending across the central forelock, mid-scalp and crown, documented within a Norwood 4A to 5 pattern.

Planning rationale: Sapphire FUE was selected for a treatment plan involving FUE-based graft extraction and recipient-site creation across a broad frontal, mid-scalp and crown treatment area. Sapphire blades were used for planned site creation, with angle, direction, depth and graft distribution determined according to the patient's existing hair pattern and treatment design.

Design notes: The frontal hairline was planned according to the patient's facial proportions, age and treatment goals, using a natural masculine contour with subtle irregularity rather than a rigid linear border. Single-hair follicular units were prioritized along the leading edge, with multi-hair grafts distributed through the temporal, forelock, mid-scalp and crown regions according to the treatment plan.

Recorded Procedure

Follicular units were harvested from the occipital donor region using FUE-based extraction. Recipient sites were created across the planned frontal, bitemporal, mid-scalp and crown regions using sapphire blades, followed by graft placement according to the intended angle, direction and distribution.

Clinical Interpretation

Documented Observation

The pre-treatment scalp record describes pronounced frontal hairline recession, deep bitemporal recession and diffuse thinning extending across the central forelock, mid-scalp and crown, documented within a Norwood 4A to 5 pattern. The recorded treatment field was the frontal hairline, temporal corners, mid-scalp and crown. The recorded procedure used 4,500 grafts. At 12 months, the clearest visual change is concentrated at the frontal hairline, with additional photographed change through the temporal corners and mid-scalp. The crown was included in the treated field; the photographs record that zone as treated and should be read zone by zone, without assuming the same visual change in every area. In the follow-up views, the transplanted areas appear visually continuous with the patient's existing wavy hair pattern. This is the documented result of one patient and does not predict another patient's outcome.
