Patient presentation and recorded decisions

Assessment & Treatment Planning

Patient goal: Improve the appearance of the previously transplanted frontal region, soften irregularities in the existing hairline and increase visible coverage across the frontal and mid-scalp areas while working within the available donor capacity.

Assessment context: Previous hair transplantation at an external clinic with unsatisfactory visible density, a pluggy frontal transition and irregular hair direction, together with continued thinning across the frontal forelock and mid-scalp documented in a Norwood 4 pattern.

Planning rationale: A combined Sapphire FUE and DHI approach was selected because the repair plan involved both broader recipient-area coverage and detailed placement around previously transplanted frontal zones. FUE-based extraction was used for graft harvesting, Sapphire recipient-site creation was used across selected mid-scalp areas, and DHI implantation was used where controlled placement between existing and previously transplanted hairs was required.

Design notes: The frontal hairline was redesigned according to the patient's facial proportions, existing transplanted hair and repair goals. The plan focused on softening the appearance of previous graft placement and creating a masculine contour with subtle irregularity. Single-hair follicular units were prioritized along the leading transition zone.

Recorded Procedure

Follicular units were harvested from the previously used occipital donor area using FUE-based extraction with conservative donor planning. Sapphire recipient sites were created across selected frontal and mid-scalp regions, while a DHI implantation workflow was used for planned placement around the frontal hairline and areas containing existing hair. Graft angle, direction and distribution were adapted to the repair design and existing hair pattern.

Clinical Interpretation

Documented Observation

The pre-treatment scalp record describes previous hair transplantation at an external clinic with unsatisfactory visible density, a pluggy frontal transition and irregular hair direction, together with continued thinning across the frontal forelock and mid-scalp documented in a. The recorded treatment field was the frontal hairline, temporal corners and mid-scalp. The recorded procedure used 4,200 grafts. At 12 months, the clearest visual change is concentrated at the frontal hairline, with additional photographed change through the temporal corners and mid-scalp. The treatment did not target the crown, so these photographs should not be interpreted as evidence of crown restoration. This is a documented repair record after previous work, not a first-time transplant sequence. In the follow-up views, the transplanted areas appear visually continuous with the patient's existing wavy hair pattern. This is the documented result of one patient and does not predict another patient's outcome.
